  • Registration: cost each client 3 RMB for registration in working time but 5 RMB in off-working time including in holidays.

  • Client need to fill relevant personal information in the Guangzhou Vaccination Handbook.

  • Client should reply doctor's questions and offer relevant information accurately and cooperate with doctor for having physical examination.

  • The doctor will give description accord with the conditions of clients.

  • Client pays the cost in the Pay-in Office according to the doctor's description, and come back to the injection room to accept treatment or vaccination injection.

  • Those who require to taking the bacterin to other service points for having injection must obey the doctor's instructions and the bacterin must be stored in refrigerator or icebox properly.

Working process of bone ashes' leaving country's conveyance

  • Applicant must bring with the decedent's cremains and relevant certificates/documents to Section of Environmental Hygiene Surveillance in GZCDC.

  • The related certificates and documents including: Death Certificate of decedent and the traveling certificate or passport of the applicant who applies to take or send the cremains abroad.

  • Hand in the inspection application: fill in the test list and pay the cost in Charging Department.

  • A leaving country's certificate will be released after the application is approved by the doctor  in the Section of Environmental Hygiene Surveillance.
